Job Description

Central is currently recruiting for a Navigator, to work in Fulham area. The service provides support to veteran’s with a variety of support needs including mental health. The aim of the service is to improve their wellbeing and independence. The successful candidate will be required to work 35 hours per week, working Monday – Friday, 9AM – 5PM. Temporary cover is required for approximately 3 months. The pay rate for this role is £14.00, with £2.04 holiday pay, totalling £16.04 per hour. Please note; for this role you will be required to work from the main service in Fulham and travel to different locations to support clients. In this position, you will be required to: * Hold a case load of clients and conduct regular key work sessions resulting in agreed support plans. * At the point of referral, work with the referral partner to ensure initial action plans are aligned with other ongoing support or treatment. * Ensure that clients access appropriate benefits advice to enable them to sustain living independently in their own homes. * Provide holistic support to Veterans, to enable individuals to access and sustain a range of activities and support within their local community * Act as advocate for individuals and support them to obtain appropriate statutory or charitable support. * Assist clients to manage their finances, by signposting services to help with financial and debt management support. * Access and obtain support from relevant mental health, drug and alcohol, counselling and ex-services support/specialist agencies. * Identify and access opportunities for leisure, employment, education and training and other purposeful day time activities for clients * Provide advice, guidance and signposting to help clients develop and maintain independent living skills. * Arrange and escort clients to appropriate activities where this is identified within action plans. * Liaise and work with other agencies including specialist veteran services, housing, health and social care professionals as required in consultation with senior staff. Please note: this role involves lone working. To apply for this role, you must have: * Experience of working with vulnerable adults in a support capacity, for example within support, employment, alcohol and drug misuse, mental health or physical disability services. * Knowledge of mental health issues, including working within a trauma informed approach. * Experience/knowledge of the issues facing the ex-Service community. * Experience of holding a caseload, carrying out needs & risk assessments, with a person centred approach. * Experience of lone working with a range of support needs * Knowledge of statutory and regulatory frameworks in adult social care. * Experience of working collaboratively with voluntary and statutory agencies, in regards to individual clients and the service. * Knowledge of best practice in regards to independent living. * Experience of networking and managing relationships with different agencies. * Knowledge of the requirements in respect of safeguarding vulnerable adults * The ability to deal with clients exhibiting difficult or challenging behaviour in a positive way * Experience of the using IT including Microsoft Office applications, CRM (Client Record Management) systems and/or online collaboration platforms Candidates must confirm their interest as soon as possible, with interviews to be scheduled shortly after and the appointed candidate expected to start as soon as possible.
