Job Description

Locum Band 6 Occupational Therapist - Community Admission Avoidance Team • Pontypridd, CF37 • Temporary/Locum contract (with the possibility of extension) • Part-time, 3 days per week • £25 per hour We are seeking a motivated and experienced Occupational Therapist to join the Community Admission Avoidance Team on a locum basis. This is an exciting opportunity for a dynamic individual to provide high-quality assessment and intervention for patients within the community to avoid unnecessary hospital admissions. Key Responsibilities • Conduct comprehensive assessments of patients referred to the community admission avoidance team. • Collaborate with multidisciplinary teams (MDTs), including nurses, social workers, and physiotherapists, to plan and implement care interventions. • Develop and implement individualized care plans based on patient needs, ensuring the focus is on maximizing independence and quality of life. • Provide therapeutic interventions in the community setting to support patients in managing their conditions and prevent hospital admissions. • Work closely with patients and their families to ensure smooth transitions and support at home. • Promote independence through the use of adaptive equipment and environmental modifications. • Offer education and training to patients, carers, and staff to enhance understanding of occupational therapy interventions. Essential Criteria • Qualification: Degree in Occupational Therapy (or equivalent) • HCPC Registration: Current registration with the Health and Care Professions Council (HCPC) • Experience: Minimum of 2 years post-graduation experience, ideally within a community or acute setting, with experience in rehabilitation, discharge planning, and admission avoidance strategies. Knowledge & Skills • Strong understanding of the principles of community therapy and discharge planning. • Ability to manage a caseload and prioritize tasks effectively. • Excellent communication and interpersonal skills, with the ability to work collaboratively within a team. • Ability to work autonomously and make decisions based on clinical judgment. Desirable Criteria • Experience in working with older adults or those with complex needs. • Knowledge of assistive technology and equipment
