Job Description
Potens Recruitment is proud to partner with West Sussex County Council to recruit an experienced Social Worker for their Older People’s Mental Health (OPMH) Team. This role offers an excellent opportunity to make a meaningful difference in the lives of people over 65 with complex mental health needs, including dementia, while working in a collaborative and supportive environment.
Key Responsibilities:
* Conduct Care Act assessments and S117 reviews for individuals with complex social care needs and mental health conditions.
* Undertake strengths-based assessments, applying relevant legislation, including the Mental Capacity Act, to empower individuals and promote autonomy.
* Develop and implement support plans aimed at improving wellbeing and achieving personalised outcomes.
* Manage a varied caseload involving complex cases, including risk management, safeguarding, and best interest decisions.
* Collaborate with multi-disciplinary teams and mental health professionals to deliver seamless care.
* Participate in joint work with health partners, including attending Court of Protection hearings and handling DOLS challenges.
What We’re Looking For:
* Qualified Social Worker with current Social Work England registration.
* Experience in completing Care Act assessments and S117 reviews.
* Strong knowledge of the Mental Capacity Act, safeguarding, and related legislation.
* Ability to work across West Sussex, with a hybrid schedule of 2-3 days in the office (Chestnuts Day Centre, Bognor).
* A collaborative mindset with excellent communication and organisational skills.
