Care Coordinator
Are you an experienced and compassionate professional with a flair for coordination and leadership in the care sector? We are seeking a Care Coordinator to join our growing domiciliary care team, ensuring seamless delivery of high-quality care services to our valued clients across Wiltshire.
About the Role:
As a Care Coordinator, you will play a key role in ensuring the efficient allocation and management of care staff, fostering strong relationships with clients and carers, and upholding the highest standards of care delivery.
Key Responsibilities:
* Coordinate and schedule care workers to meet client needs in a timely and efficient manner.
* Conduct regular communication with clients, carers, and other stakeholders to ensure smooth service delivery.
* Monitor and manage care worker performance, rota, attendance, and compliance.
* Support recruitment, onboarding, and training of new care staff.
* Liaise with social workers, health professionals, and families.
* Manage client hours and invoicing management
* Assess newly admitted client, prepare care plans and ensure adequate risk assessment documentation and recording.
* Maintain accurate records and ensure compliance with CQC standards and company policies.
What We’re Looking For:
* Experience in a similar role within the domiciliary or healthcare sector.
* Strong organisational and time management skills.
* Excellent interpersonal and communication abilities.
* Proficient in using care management software and Microsoft Office.
* A team player with a proactive and problem-solving mindset.
* Flexibility to work occasional evenings or weekends if needed.
