Experience within Social ServicesNonStop's client is a local authority based near Wembley is seeking a Social Care Assessor to join their Social Work Adults Learning Disabilities Team. You will deliver high-quality care to the wider community. This will mean producing high-quality work in terms of the Care Act and associated social care legislation with the ability to respond promptly to crises as well as hospital discharges.
Your duties will include:
Carry out social care and joint health assessments.
Devise appropriate comprehensive support plans in order to meet identified needs
Mmonitor and review packages of care
Undertake actions in relation to Safeguarding Adults, as required within the locally agreed Adult Safeguarding policiesBefore You Apply
