Role: Adults QSW - Home to Decide Team Type: Locum - Full time hours Rate: up to £32.65 Eden Brown are recruiting on behalf of South Gloucestershire County Council who are looking for a qualified Adults Social Worker to join their Home to Decide Team. This is a full time, locum role to start as soon as possible. Supporting discharge from our acute hospital and community team we require a social worker who is able to complete fast paced work. Complexity in hoarding, mental capacity assessments and supporting long term care needs. Working in a MDT with OTs and social care practitioners. - Initial assessments in persons own home - Review and monitor working towards goals - Identify if there are long term care needs - Use of digital system ie mosaic. - Predominately based in the office and within South Gloucestershire County.
